As Trent said, this is pretty easy with something like the Gimp.
(Or even in emacs, with hscroll-mode and rectangular cut & paste ;-)

This patch to data/default/tiles.xpm moves the hitpoint
bar from the top left of the tile to the bottom center.
(That is, so as to not obscure the flags.)   

I also reduced the black border of the hp bar (to just 
one pixel), and made the 'A', '+' and 'X' symbols slightly 
narrower, so that the hp bar fits better.  (The X still 
overlaps the hp bar slightly, and it would probably look 
better if the X were drawn over the top of the hp bar, 
rather than the other way round.)

I think it works quite well.
