Just yesterday I was playing with an old Slackware 3.0 installation on
an even older machine at work.  Had lots of interesting errors until I
found I had grabbed a couple of old floppy disks and had quick-formatted
them under DOS.  Fixed that little detail by reformatting a couple of
new diskettes with the regular format.  Moral, if there's any bad
sectors on a floppy disk anymore, I just pitch 'em.

Might it be possible there is some problem with the installation media,
or the floppy drive?  Are you creating the boot disks on the target
machine or another machine?  All the hardware I have access doesn't
support booting from a CD-ROM, so I don't have any ideas there.
