I couldn't say what the problem is, but here's my setup for fastmail
syncing:

[Account FastMail]
localrepository = Local
remoterepository = Remote

[Repository Local]
type = Maildir
sep = .
localfolders = ~/.maildir

[Repository Remote]
type = IMAP
ssl = yes
sep = .
remotehost = mail.messagingengine.com
nametrans = lambda foldername:
re.sub('^$','.',re.sub('^INBOX','',foldername))

II guess you'd have to another localfolders (Malidir), but other than
that, the above should work for fastmail,
