That is just simple foolishness. It should always be possible to enter
an unoccupied city, or the code should be changed to allow this as a
serious bugfix :-).

Thus, you only need to pulverize the place before you go in.

And diplomats should not be restricted from diplomatting by attack
based rules, which allows you to buy the place out, or sabotage its
sabotageables. Similar things could be said about caravaning to 
such places.

Most and maybe all real CIV versions allow such things.

Having said this, the protection from land-based attacks afforded
by tiny islands is at least partially offset by the lack of growth
resources that don't really change until Offshore Platforms are
available, so it isn't necessarily a one-way benefit. And Civ is
all about strategy and benefit tradeoffs.
