I cannot reproduce this.  As always, do you have a savegame which

Yes, this is a longstanding problem, that a very simple method 
is used to choose the suggested name:  From the list of names 
for that nation, the first name is chosen which is not the name
of any existing city.  So if there is never a city named 
"Toulouse", it will forever suggest "Toulouse" as the next name,
as you found.
