You must pay for:

1. A regular phone line
2. DSL service
3. ISP service

If you currently have two phone lines (one for Internet access), you can
cancel one of them, and run DSL over your main phone line

DSL service is $39/month for me for the 1.5mb/128k plan.  I don't know if
it's more for the 6mb/1.5mb plan or not (I would guess it probably is). 
Then ISP service for a signle dynamic IP seems to be ~$10 (from either SW or
SWBI).  If what Clint says is correct, you get free ISP service from SWB if
you commit to a year service, with their current promotion.
