OK, Miguel, I'll bite.

The oldest reference I can find for "Hello, World" is K&R.  It seems
like an obvious thing to do, but is there any evidence of its being used
as the canonical test program before C?

The README file for GNU Hello says:

# The basic algorithm is described in: "The C Programming Language", by
# B. W. Kernighan and D. M. Ritchie, Prentice-Hall, New Jersey, 1978;
# the program is an enhancement of the one published in that book.

There is no mention of earlier implementations of the `algorithm'.

If RMS is still lurking I'm sure he knows . . .
