I think you can have multiply unhappy citizen(in civ1 and civII), so that in
a city of size 2
(h=happy c=content s=scientist/taxman e=elvis 1=unhappy 2=twice unhappy)

in a big empire a new city founded far from the capital that has grown once
looks like:
21
with an entertainer:
1e   (and starving ..)


Note: definitely in civII, (double) unhappyness doesnt affect all cities,
but only some.
