I don't know af any good IDE for that yet, but vim has a very good 
highlighting scheme for any php document to assist with the structure of 
your coding.

Abook I found quite useful was MySQL/PHP Database Applications by Jay 
Greenspan & Brad Bulger.
It is a bit dated now, but is still quite useful.    I also use the 
online documentation for mysql and php extensively.

A currently used and freely available MySQL/PHP application that can be 
looked at to see what it is doing with the database and dynamic 
templates/style sheets is PNPNuke from www.phpnuke.com ,
