I believe that (a) and (b) are incompatible.  I've never encountered a
reliable modem for under $50.  (And having spent three years working at
SouthWind, I've dealt with a wide range of modem problems.) 

You can't go wrong with a 3COM/USR, but you won't get a new one for much
under $100, if that.
