First a message to all -- I have been swamped with other things over the
holidays, but will now try to get back into the swing of things
OfflineIMAP-wise.

First things first -- can you upgrade to 3.99.5?  I believe that this will
at least fix your CRAM-MD5 problem.

It seems that your TCP stack is not recognizing when the connection goes
down.  It should detect this condition after a few minutes (at most), and
send back an error code to OfflineIMAP.

Stranger, though, is that you have holdconnectionopen = no.  As long as the
connection goes down between syncs, this should not cause any problem.

Do your DNS servers change when your connection resets?
