I was thinking about this problem before your post. We also have a 
problem with successor states. For example, should we have London AND 
Londinium (the Roman name)? The results can be peculiar.

One way to have both would be to record aliases for city names. The AI 
could refrain from choosing a city name if one of its aliases was 
already chosen.

For example, by recording names like this: "Londinium|London (ocean,river)".
