Because the Mordor location list is really "Evil places of Middle-Earth", 
and needed more members a lot more than the Numenor list did.

Because all the leaders are historical/legendary.  Admittedly this could
have gone the other way.

Moontower was a poetic name of Minas Ithil.  For the others, see above.
