I put together one single patch for naming cities with respect to terrain
type. This one only implements the Canadian ruleset, but does it
thoroughly. Cities should be appropriately named if built

 * by a human
 * by an AI
 * from a hut

The patch knows about hills, rivers, and coastal tiles; this covers the
majority of English city names, and probably other languages as well.

This is just a combination of the previous patches (except only the
Canadian ruleset is modified). This successfully patches on March 19th.

Please feel free to modify the ruleset for any nation for which you
understand the language.
