Vasco and I discussed the problems with the CMA dialog and came up with 
a system similar to the worklist system.  CMA presets are editable via a 
global dialog, while the CMA tab of the citydlg provides only a list of 
presets to choose from (or none).  The "apply once" button is gone 
altogether.

The advantage of this is it makes the dialog interface simpler 
(particularly for newbies who can use the provided presets and simply 
choose one).  The disadvantage though is that you can no longer fiddle 
with a city's sliders directly: you have to open the CMA editor dialog 
and edit the preset directly.  In other words there would be no such 
thing as a "custom" or anonymous CMA setting.
