That is what Generator 3 is meant for.
The doc was changed after failure to restore original mapgen.
Please search the archive 1-2 year back and check the RCS version log.
Anyway, the latest version,
http://arch.freeciv.org/freeciv-dev-200007/bin00001.bin
makes it possible for users to set whatever kind of small islands
they want.
